Early Positive Pregnancy Test: How Long After Implantation?

An early positive pregnancy test  becomes positive only after the fertilized egg has first implanted into the uterus and then sent enough amounts of hCG, human chorionic gonadotropin, the pregnancy hormone, into your blood stream and from there into the urine.

Implantation, when the fertilized egg implants into the endometrium, happens about a week after ovulation (range: 6-12 days), and it takes another 3-4 days after implantation (9-16 dpo: days after ovulation) for the blood pregnancy test to first become positive, followed 2-3 days later by the early positive pregnancy urine test, the home pregnancy test (HPT).

  • Blood Pregnancy test: If you are pregnant then the blood pregnancy test is usually positive within 3-4 days after implantation or about 9-10 days after fertilization and ovulation.
  • Urine (Home HPT) Pregnancy Test: The early positive urine pregnancy test  will usually not become positive until 2-3 days after the blood test, and in most women until a missed period or about a week after implantation. Many manufacturers of urine pregnancy tests want you to believe that the urine test is usually positive before a missed period, but only about 25% of pregnant women will test positive with the HPT 2 days before a missed period, and about 40% the day before the missed period.

The wide variation of when the urine pregnancy test becomes positive and how early pregnancy can be detected are due to four variables which determine when the home urine pregnancy test becomes positive:

  1. Timing of Implantation
  2. Blood hCG Level
  3. Urine hCg Level
  4. Sensitivity of the Urine Test

1. Timing of Implantation
A pregnancy test detects the presence of the pregnancy hormone hCG (human chorionic gonadotropin) either in the blood or in the urine. The hCG in the urine comes from the hCG in the blood after it was filtered through the kidney. After ovulation, the egg gets fertilized and travels through the fallopian tube toward the uterus. This travel takes an average of a week. Implantation, the attachment of the fertilized egg in the lining of the uterus, occurs on average about a week or later after ovulation/fertilization, but it can happen as early as six days and as late as 12 days after fertilization/ovulation. At the time of implantation the placenta starts making the pregnancy hormone hCG, which then enters the blood stream. How early pregnancy can be detected depends on the pregnancy hormone hcG which can be detected in the blood about three to four days after implantation. Urine hCG can be detected about two to three days after blood hCG can first be seen. Detection of hCG depends on timing of implantation, so hCG can normally be detected in the blood between nine and 16 days after ovulation (HPT: 12-19 days after ovulation).

2. Blood hCG Level 
There is a wide normal variation of hCG levels, both in the blood and the urine and this wide range changes the diagnosis of how early can pregnancy be detected. If a woman has low blood hCG levels, for example, then it can take several more days for it to show up on a pregnancy test.

3. Urine hCG Level 
Urine hCG levels change over the course of the day depending on how much you drink and how diluted or concentrated the urine is. Urine pregnancy testing is best done with more concentrated urine. So how early can pregnancy be detected depends on the urine concentration and the quality of the early positive pregnancy test. If you drink a lot of fluid, the urine may be too diluted early on in pregnancy to achieve an hCG concentration adequate enough for a positive test.

4. Sensitivity of the Early Urine Home Pregnancy Test
Different pregnancy tests have different sensitivities and that has an impact on how early can pregnancy be detected. The lower the sensitivity, the earlier a pregnancy test becomes positive. Sensitivities are indicated in mIU/mL, the lowest amount of hCG in the urine that tests positive. A sensitivity of 20 mIU/mL requires one-half the hCG level to be positive when compared with a 40 mIU/mL sensitivity level, and it may become positive several days earlier than the pregnancy test with a higher sensitivity. Make sure you read the package of the pregnancy test to determine your pregnancy test's sensitivity.

good morning doctor, i've made I.u.I on 24/10 and on 7/11 i made a hCG blood test and the result was 1.99 and i didn't have my period yet does that means that i'm not pregnant or there is a chance that the level of the hCG will get higher ?? waiting for your answer thank you

i have heavy cycles that last 5 days i went to the obgyn who advised me of my uterus measuring at 11wks could be a fibriod. the following week my cycle came on that wednesday heavy but not as usual then thursday was lighter then wednesday on friday i was able to wear a panty liner due to just spotting sat & sun nothing. monday i did a hpt negative......this week i did one negative

 i have a question..my cycle is 32 days nd my first day of last period was 12 aug. i had un protected sex wid my hubby on 21 aug. i have cramps on my lower abdominal pain since 2 days ..nd i have tenderness in my breast for 3 days..could i am pregnant coz i want baby..???can u pls reply me

hi doc, this has been of much trouble to me. I saw my period last on the 18th of this july, 2011 and on 17th August it tested tve with urine hcg test then on 18th August, 2011, I started cramping with some spots. What could be the problem and how do I stop the flow in it comes heavier?   my cycle is 28 day.

i feel kinda stupid asking this/writing this online. but i think i may have gotten pregnant and was wondering what you think. okay i had my period the week of June 5th and on the 18th me and one of my friends were partying togethor and hooked up i though we used a condom but a few days later he informed me we hadn't . I'm pretty sure that was the weekend i was ovulating since I've always been regular with my period. right now it would be too early to take a test to see if i am and was hoping to see what you think......

So, I have always had irreguar cycles which has been "normal" in my family. I was pregnant once before and I ended up miscarrying. Very hard experience on me and my husband, I was going nuts looking at baby things and watching baby shows, and paying attention to all my pregnant friends. Once my husband and I started TTC again I was happy and at ease... lets just say I wasn't crazy anymore... I was willing to let it happen on its own time. I recently had my period. First day was March 2nd. It is now March 29 and for the past week I have had pregnancy symptoms... almost the exact same ones as my first one with a few differences. Constantly tired, very sore breasts with swelling to the point my husband can tell a difference, pressure in my stomach, dizzy or lightheaded feeling, random headaches that come and go (like with my first pregnancy), and now I am experiencing feeling sick and almost naseous after eating. With my first pregnancy I knew the exact day I had gotten pregnant (or implanted) ... I could sense it somehow and my body felt different so within a couple weeks I tested positive. With this one, (if it is one), I have an idea of when I may possibly have gotten pregnant, but so far the urine tests are negative. I am using the Early First Response, but I can't tell if I am taking it too early or not due to the fact that I never know when my period will actually show up so I just take it whenever. I have read online that this can be "all in my head" but I don't see how because as soon as my husband and I started TTC again I let all my crazy thoughts about getting pregnant just go. Infact, I hardly ever think about it anymore. Do you think that a urine test after only about a week of pregnancy symptoms is too soon to get a positive on that First Response that I used? Should I go get a blood test to confirm?

Hi, my last period started on the 24th of January and lasted 6 days, me and my partner had sex when I ovulated (around the 7th) and I have had two clear positive tests this morning... I'm not due on until the 21st so will this be correct? *fingers crossed it's true*. Thanks in advance
